Disc spring sags after several individual usages

What are the possible reasons to cause a disc spring sags after several individual usages? I think it might be heat treatment incompatibility, but in what cases (high temperature? low temperature?) does this happen?

Spring Newbie, if the disc springs fatigue after several individual usages, the applied load may be too large or they were defective products!
In addition, when the temperature of the operating environment rises, the allowable stress decreases, so the springs are easier to fatigue than in a normal temperature environment.
Moreover, if an error happens during the heat treatment process such as quenching and tempering, the springs may not have sufficient hardness. By measuring the hardness of the material, it is possible to easily check for heat treatment’s defects.

When it comes to manufacturing defect that leads to the fatigue of the springs, it is possible that the springs’ strength is insufficient due to poor heat treatment!
In addition, although it is not a manufacturing defect, if the springs used did not go through setting, initial fatigue may occur when the springs are used!
